Together with the Massachusetts Department Public Health, the Mass Partnership for Healthy Communities (a program of Health Resources in Action) is pleased to announce the third year of the Healthy Communities Principles Awards. These awards will be presented at the Fifth Annual Healthy Community Conference in Worcester, MA on June 21, 2011.
The awards recognize the work of community coalitions/teams which best exemplify at least one the nine Healthy Communities Principles in action in their community.
The purpose of these awards is to highlight creative and useful best practices developed by communities as they implement the Healthy Communities Principles in their work in order to better promote their use throughout the Commonwealth.
If you feel that your community coalition/team is a model for how to implement a Healthy Community Principle (or two!) in your community work, we invite you to nominate your coalition or team for the corresponding award. The awards selection committee will be made up of healthy communities advocates. A community may be granted up to one award per year.
Please complete the below form to tell us each nomination (limit 2 per community coalition): What were your model achievements in implementing the identified principle and why your coalition deserves recognition for this work? We encourage brief descriptions and suggest that you edit your responses in a text editor, such as Microsoft word, and then copy and paste them into the form.
